Thank you for choosing Mind Body Skin Medspa. We believe that part of any good healthcare practice is establishing and communicating a financial policy. We are dedicated to providing the best possible care for you and we want to make sure you fully understand our policies.

billing policy

  • Mind Body Skin Medspa does NOT participate with or submit billing for any private or government insurance companies. There are times when our providers use diagnosis codes, and in these cases we can provide you with a Super Bill upon request so you are able to complete the appropriate forms for your insurance.
  • Lab Tests and/or Pathology Specimens sent to outside laboratories will be billed separately from Mind Body Skin Medspa.

A credit card may be required to reserve an appointment for treatment scheduled in advance.

grouped treatments

For specially packaged or grouped treatments, payment for the entire package is due at the time of the first scheduled treatment.

used individually or combined according to your desires:

  • CASH (We DO NOT accept personal checks)
  • MAJOR CREDIT CARDS: VISA, MasterCard, American Express and Discover

refund policy

ALL SALES ARE FINAL. Before a service is performed please consider all of the required protocol and side effects. We are committed to patient satisfaction and are available to answer any questions or concerns you may have with regards to the services we offer before you make a purchase. Mind Body Skin Medspa may provide patients with prescription medications, and these medications are subjected to state and federal laws. These laws do not permit us to restock sold items and accept returned prescription medications or treatments for refund.

THERE ARE NO REFUNDS FOR SERVICES ALREADY RENDERED. If a package or series of treatments has begun, these services will be considered to have been rendered even though the full series may not have been completed. Should you wish to discontinue your treatment during a series, credit for the unused treatments at the discounted package price may be extended and may be used to purchase other treatments or products offered by Mind Body Skin Medspa.

CANCELLATION POLICY

The most valuable thing you can give someone is your time, and we fully believe that everyone’s time should be respected. We understand that a situation may arise that could force you to cancel or postpone your treatment. Please understand that such changes affect not only our staff but our other patients as well, and we therefore request your courtesy and concern. If you need to cancel your appointment, please notify us of the cancellation at least 24 hours prior to your scheduled treatment time. Should we receive less than 24 hours notice, or should you fail to keep your appointment, the following may result

CANCELLATION POLICY
  • Late Cancellations –Those attempting to cancel within less than 24 hours of the appointment time will be charged 50% of the treatment scheduled.
  • "No-shows”- Patients who are not present for their appointment will be considered a no-show and charged 100% of the treatment scheduled.
  • Patients may also be required to hold future appointments with a credit card that will be charged in the event of another no-show or late cancellation.
  • A continued history of no-shows may result in a dismissal from Mind Body Skin Medspa. Consultation visits that are repeatedly cancelled will count towards the patient’s no-show record and may result in refusal to schedule future appointments. In accordance with state guidelines, a patient may be discharged from the practice following three (3) no-shows in a one-year period (365 Days)

REVISIONAL TREATMENT OR TREATMENT OF COMPLICATIONS

The practice of medicine and surgery is not an exact science, and medical spa treatments are the practice of medicine. Although good results are anticipated, there can be no guarantee or warranty, expressed or implied by anyone, as to the actual results you may get. Additional treatments and/or treatment for problems or complications may be required. These could result in additional charges for which you may be responsible. Your insurance, if you have it, may or may not cover the expenses related to actual complications or other medically-related problems arising out of treatment at Mind Body Skin Medspa.
